Compensation Disclosure Form

Effective January 1, 2009, all insurance producers soliciting applications for health care insurance shall disclose to the applicant that the insurance producer will receive a commission.

Anthem	20% first year, 5% renewal years
Assurant	20% first year, 5% renewal years
Delta Dental	8% premium per month
Kaiser Permanente	$13.50 per Subscriber per month

Group Health Insurance

Aetna	4% of Premium per month
Anthem	$25 per Employee per month
Kaiser Permanente	$25 per Employee per month
